Shortly after take-off Ił-62M SP-LBG „Tadeusz Kościuszko” airplane, flying over Warlubie with 183 people on board, encountered a technical failure. As a result of the explosion of engine number 2, also engine number 1 was damaged. The reason for the explosion was the wrenching off of the turbine shaft in one of the engines.

2 minutes before the crash the crew lost control over the aircraft. The plane crashed 5 kilometres away from the edge of runway 33 of Warsaw Okęcie Airport. None of the passengers and crew members survived the crash. Till the last moment the pilots tried to safely land the airplane. Their last words addressed to the approach control in Warsaw became the symbol of the tragedy: „Dobranoc! Do widzenia! Cześć! Giniemy!” (Goodnight! See you! Bye! We’re dying!”)
